At the heart of Caregiver Homes is unwavering support for caregivers. Our model of care is designed to provide you with everything you need to make a full-time commitment to caring for an elder or individual with disabilities. Once you qualify for Caregiver Homes, a dedicated care team, including a registered nurse, will assess your family’s specific needs and create a plan of care for your situation.


You and the care team that works together to provide truly personalized care. Caregiver Homes is structured to include regular visits from care team members. Each day, you will fill out a quick electronic report in our easy-to-use Caregiver Homes Care Management System to ensure that everyone involved, including the participant’s doctor, can ensure high quality care. To further support your commitment to full-time caregiving, the state Medicaid program will provide you, through Caregiver Homes, with a modest daily cash stipend. As a qualifying Adult Foster Care payment, this stipend is not taxable and will not impact other cash benefits you or your family may be receiving.


What does it take to be eligible for Caregiver Homes?

To be eligible for Caregiver Homes, the consumer must be eligible for Medicaid, need 24-hour, live-in care, and require help with one or more of the following personal care needs:


  • Bathing
  • Dressing
  • Walking
  • Transferring (helping getting in/out of bed)
  • Toileting
  • Eating
